Beneficiaries of employees are eligible for a refund of employee contributions in the event of the member's death. All employees should designate their beneficiaries through Retirement Online. (See the ROL see link below).
To log into Online Services, the employee must create a User Profile. To gain access to create a profile, the employee uses his or her social security number and date of birth.
If you do not have online access you may download the applicable form linked below and mail or fax to the address at the top of the form.
